A specialist review can be much smaller than an advanced-therapy episode
Some international patients need confirmation of the diagnosis, medication optimization or a second opinion. Others are being assessed for pump therapy or DBS. These pathways should not be compared as if they were the same hospital service.
Medication and diagnostic review have their own cost drivers
A specialist evaluation can include neurological examination, review of medication response and fluctuations, imaging or additional diagnostics when needed, cognitive or neuropsychological testing in selected cases and development of a revised treatment plan.
Pump therapy involves device, initiation and follow-up planning
Apomorphine or levodopa-gel pump pathways can require suitability assessment, device or access-system costs, medication initiation, training, dose adjustment and follow-up. The total depends on the exact system and whether the initiation is outpatient or inpatient.
DBS is a separate surgical and programming episode
A DBS quotation can include candidacy assessment, imaging, neurosurgical implantation, the pulse generator, anesthesia, inpatient stay and later programming. Long-term programming and battery or generator replacement should be distinguished from the initial operation.
Budget for follow-up after returning home
Parkinson treatment is longitudinal. Before choosing an advanced therapy in Germany, confirm who will manage medication changes, pump supplies or DBS programming near home and which follow-up services are included or excluded from the German estimate.
