Confirm why bladder removal is being recommended
Freiburg and Heidelberg describe radical cystectomy for invasive or otherwise high-risk bladder tumors. Before international travel, ask the German team to state the tumor stage, pathology and reason cystectomy is preferred over continued endoscopic/intravesical management or another bladder-preserving strategy in the individual case.
Robotic surgery changes access, not the cancer indication
Heidelberg and Freiburg document Da Vinci-assisted cystectomy. Robotic assistance may change the minimally invasive surgical approach, but it does not eliminate the need for complete oncologic resection, lymph-node assessment when indicated, reconstruction and management of major-surgery risks.
Urinary diversion must be planned before surgery
Freiburg documents ileal conduit, neobladder and other reconstructive urinary-diversion options. Suitability depends on cancer location, urethral and surgical margins, kidney function, bowel status, manual and cognitive ability, prior radiotherapy, patient priorities and the center's assessment.

The hospital episode can be materially larger than the operation itself
The treatment plan can include preoperative optimization, anesthesia assessment, surgery, urinary reconstruction, pathology, inpatient monitoring, pain control, bowel recovery, thrombosis prevention and management of complications. International patients should ask for the expected inpatient stay and the minimum period they must remain near the center after discharge.
Prepare for long-term functional follow-up
A urinary diversion changes daily care after cystectomy. Before returning home, clarify catheter or stent removal, stoma or neobladder training, fluid and electrolyte monitoring, infection warning signs, pathology review and the oncology surveillance schedule.
