German university hospitals generally calculate an international-patient estimate after the responsible clinical department reviews the case and defines the expected diagnostics or treatment. The estimate is provisional because the final invoice depends on the care actually delivered.

Cost drivers in a PCI admission
The final episode of care can include several components beyond the stent itself.

- Number and complexity of coronary lesions treated

- Additional intravascular imaging or physiology when used
- Observation or inpatient stay
- Treatment of complications or unexpected findings
Compare the clinical plan, not only the headline amount
Two quotations are only comparable when they assume similar diagnostics, intervention complexity and length of stay. Ask each hospital what its estimate includes and excludes.
