Atrial fibrillation (AF) is a common sustained heart-rhythm disorder, but treatment is broader than a single ablation procedure. The 2024 ESC AF-CARE framework starts with management of comorbidities and risk factors, then prevention of stroke and thromboembolism, symptom control through rate or rhythm strategies, and repeated reassessment as the patient's condition changes. Catheter ablation, cardioversion, antiarrhythmic medication and selected left-atrial-appendage closure each answer different clinical questions.
Confirm the rhythm diagnosis and clinical context
A specialist review should establish that atrial fibrillation has been documented and identify the pattern, symptom burden, ventricular function and associated cardiovascular disease before a treatment strategy is chosen.
- 12-lead ECG documenting AF when available
- Holter, event-monitor or wearable rhythm reports
- Echocardiography and ventricular-function assessment
- Previous cardioversion or ablation reports
- Current medication and anticoagulation history
- Stroke/TIA, bleeding and heart-failure history
- Blood pressure, diabetes, kidney function and other major comorbidities
Stroke prevention is a separate decision from rhythm control
The need for oral anticoagulation is assessed from the patient's thromboembolic risk and clinical context. Restoring sinus rhythm with cardioversion, medication or ablation does not automatically remove the need for stroke-prevention treatment, so anticoagulation decisions require ongoing specialist reassessment.
Rate control and rhythm control answer different goals
Some patients are managed primarily by controlling the ventricular rate and symptoms, while suitable patients may benefit from a rhythm-control strategy using cardioversion, antiarrhythmic drugs or catheter ablation. The choice depends on symptoms, AF pattern and duration, structural heart disease, comorbidities, previous therapy and patient preferences.
Catheter ablation is one rhythm-control pathway
Catheter ablation, commonly based on pulmonary-vein isolation, can be used as a rhythm-control strategy in selected patients. German electrophysiology centers use radiofrequency, cryoballoon and increasingly pulsed-field technologies, but the energy source should not replace the more important decision about whether ablation is clinically appropriate and how follow-up will be organized.
Left atrial appendage closure is not a routine substitute for anticoagulation
LAA closure can be considered in selected patients after specialist assessment of stroke risk, bleeding history, anticoagulant suitability and appendage anatomy. It is a different clinical pathway from AF ablation: one primarily addresses thromboembolic protection, while the other is a rhythm-control procedure.
Risk-factor treatment affects the whole AF pathway
Management of hypertension, obesity, diabetes, sleep apnoea, alcohol intake and other modifiable factors is part of AF care and can affect symptoms, recurrence and the success of rhythm-control treatment. International treatment planning should therefore include long-term care after returning home rather than focusing only on the procedure date.
